To be eligible for Diagnostic Center Services, a student must be enrolled in a special education program and:
REFERRAL INFORMATION
- making inadequate progress despite local school efforts, or
- presents a complex learning profile, or
- require additional diagnostic information to assist in defining the most appropriate educational goals and teaching strategies.
Students whose primary disability is Hard of Hearing or Deaf should be referred to the California School for the Deaf Assessment Center. Those with vision impairments as the primary disability should be referred to the California School for the Blind Assessment Center.
Students must be referred to the Diagnostic Center by their Local Educational Agencies (school district or county office of education). Districts must have exhausted their local resources in an attempt to answer the diagnostic questions prior to submitting an application for services. The initial referral should include:The records are screened by an admissions committee. The Local Education Agency and parents are notified of the status of the application. If accepted, an assessment plan is developed and an assessment date scheduled.
- a completed application packet specifying district/parent concerns, diagnostic questions to be addressed and district attempts at remediation
- the most current Individual Education Plan
- the results of the most current evaluations (reports older than 2.5 years will not be accepted)
- release of information forms dated and signed by parent
- authorization by LEA Director of Special Education
