4 Steps to transition planning:

Look at the big picture when answering

“Who is the student?”

 

Step l: What is the present picture?

Look at: important events in the student’s life, strengths, favorites, friends, community activities, living arrangements, school abilities, and transportation.

Step 2: What is the future picture?

Look at: where student would like to live, what supports he/she will need to manage work - money - transportation, and how student will advocate for him or herself.  

Step 3: How does big picture planning look?

Look at: high priorities, solutions/problem-solving, available resources, and making best choices.

Step 4: What is the action plan?

Look at: sequence solutions, determine specific resources, develop plan and take appropriate action, evaluate and revise plan as necessary.
